How to Create a Questionnaire

This document explains how to create a questionnaire for surveys, determining meeting times, etc.


1. Go to the directory in which you’d like the questionnaire to show.
2. On the top edit navigation bar, select “view”.
3. Select “add items” and then select “questionnaire.” 
4. Fill in the necessary fields for each question (each question will be created one at a time and shown at the top view for editing as needed). 
5. Click on the "view" menu to see what you've created before publishing/submitting for review.
